I found several nostalgic mentions of the "Colonial House Restaurant" in Salem, OR, but not even a mention of their thousand island dressing, much less a recipe for it.
There are a couple of Facebook pages dedicated to Salem, Oregon nostalgia, and the Colonial House Restaurant is mentioned on both of them. If anyone at all has the recipe and visits those sites, then they might respond if you post your request there:

Also, this person lists the Colonial House in Salem, Oregon as a place that he worked. He worked in the kitchen there, so he might know the dressing recipe. Post your request on his page and he might be able to help:

Mayonnaise, ketchup, cream, and garlic powder. No pickles.
We mixed it in big batches. I can’t say what the proportions were.
I was head cook there at one time.
