I searched for this every way that I could think of. I found multiple requests for a cookie recipe from the Post Oat Fortified Flakes box, but no one had received a positive response. I had no success at all finding any cookie recipe calling for oat flakes cereal and chocolate chips.
I found several photos of Post Fortified Oat Flakes boxes, including two photos showing the back of the boxes, but neither of those had a recipe on the back of the box.
I did find one photo of the side panel of a Post Fortified Oat Flakes box that has a readable oat flakes cookie recipe, but the recipe does not call for chocolate chips. See here for the scan: Mr Breakfast
Perhaps your mom used this recipe and added the chocolate chips on her own? I have transcribed the recipe below. Note that the photo/scan is visually unclear regarding the amounts for the soda and the white sugar. In transcribing the recipe, I have used my best guesses and I consulted other, similar recipes, but ultimately I am uncertain as to the exact amount that the recipe calls for in those two ingredients. I am confident in the amounts for the other ingredients.

Oat Flake Cookies 1 cup sifted flour 1/2 teaspoon double-acting baking powder 1/2 teaspoon salt 1/2 teaspoon soda 1/3 cup butter or other shortening 1/2 cup sugar 1/2 cup firmly packed brown sugar 1 egg 1 teaspoon vanilla 3/4 cup Post Oat Flakes Measure sifted flour, add baking powder, salt, and soda, and sift again. Cream butter. Gradually add sugars, creaming until light and fluffy. Add egg and vanilla and beat well. Add flour mixture and cereal. Mix thoroughly. Shape dough into small balls and place on ungreased cooky sheet. Bake in moderate oven (375°F.) for 12 to 15 minutes.
